Services

The spectrum of our services ranges from wholesale turnkey sales support for agents including quoting and plan illustrating and sales collateral to rating, underwriting, enrollment, eligibility, case administration, claims administration, customer service, commission payment, agent licensing, cash reconciliation, billing and billing customization. The majority of our services are web-enabled, allowing agents, employers and employees to administer plans online, and track the progress of outstanding claims easily and instantly.

Human Strength

JEFF W. BAK

President and Chief Executive Officer

Jeff has spent over 15 years in the insurance, reinsurance and healthcare services industry. Jeff joined HealthPlan Services in January 1995 and served in various sales and business development leadership positions. In May of 1999, he became President and Chief Operating Officer while spearheading all operations of HealthPlan Services, Inc. Then in 2000, Jeff led the buy-out of HPS, ABPA and Montgomery Management. In 2001, he was named President and CEO of HPH and has presided over the company's consistent revenue and earnings growth.

Jeff began his career with Dun & Bradstreet in 1989 as a sales management trainee. He then became a Sales Director for Dun & Bradstreet. In 1992 he left to work as a reinsurance broker with Peglar & Associates, a boutique reinsurance intermediary, in Stamford, Connecticut through 1994. Jeff is a graduate of Clemson University where he earned a B.S. in Business in 1989 and completed the Advanced Management Program at the Kellogg School of Business at Northwestern University in 1999.

ARTHUR J. SCHULTZ

Executive Vice President, Chief Operating Officer

Before rejoining HealthPlan Services in February of 2001,Art was Senior Vice President at My Way Health in Oakland,CA, an internet-enabled health care start-up company where he contracted to partner with founders of companies to establish strategic focus and develop product offerings.

From 1998 to 2000, Art was a consultant at Arbella Mutual Insurance Company, the second largest property and casualty insurer in Massachusetts with 1000 employees. Art was employed at HealthPlan Services from 1996 to 1998 as Executive Vice President. This position resulted from the acquisition of Consolidated Group, Inc. where he was a minority owner and Chief Operation Officer from 1986 to1996. Consolidated Group was, at the time, one of the largest privately held, third party administrators with an employee base of 750. He also served during this same period of time as President of Consolidated Group Claims, a wholly owned subsidiary of CGI with 250 employees.

ANNE MARIE FARIA

Executive Vice President, Sales Development

Anne Marie began working with HealthPlan Services in 2001.She has extensive experience developing new manufacturing and distribution network relationships. She has created agency distribution networks that use a common platform service model to distribute products such as Health, Life, DI, LTC and other employee-benefit products suited for the individual and small business marketplace. Prior to working with HPS, Anne Marie began her career at New England Financial (NEF) in 1974. Over her 26-year career, she has managed a variety of organizations including: Human Resources, Small Business Marketing and Sales, the Small Group Health Program and most recently Product Management for Life, Annuity, Long-Term Care, Disability and Group Insurance. Anne Marie worked closely with HPS during her tenure at NEF. She retired from NEF in Fall 2000.

Ed Forth

Senior Vice President / Chief Information Officer

With over 21 years of IT experience, Ed has spent over 14 years in the insurance and healthcare services industry. He joined HealthPlan Services in April 2007 focusing initially on all IT Applications Development including formalizing standard processes such as Software Development Life Cycle (SDLC).

Ed started his career with BlueCross BlueShield of Tennessee in 1986 as an applications developer in the mainframe applications division and then left in 1990 to work as a functional manager of mainframe and client/server applications with Great Western CFG, a financial services company. After advancing to Director of Production Support and Applications Development, Ed left Great Western to become CIO at Comprehensive Care Corporation, a carve-out behavioral healthcare payer. Finally before joining HPS, Ed spent 8 years as a healthcare management consultant for PricewaterhouseCoopers and Comdyn Inc where he consulted for major customers such as Cigna, Pharmerica, BlueCross BlueShield of Tennessee and the Blue Cross Association. Ed is a graduate of Tennessee Technological University where he earned a B.S. in Computer Science in 1986 and a graduate of Central Michigan University where he earned a M.S. in Business Administration in 2006.

THOMAS CHADWICK

Chief Financial Officer

Tom joined HealthPlan Holdings in August 2005, and brings25+ years of financial, strategic and operational leadership experience from a broad range of businesses and operating environments.

He most recently served as CFO at Provox Technologies,Inc., a venture-backed healthcare information technology company providing speech recognition-based workflow solutions to hospital, clinical, and physician practice clients. From 2001 to 2004, Tom was CFO of Misys Physician Systems, the largest division of Misys Health care System sand a major provider of practice management systems and transaction services to healthcare providers. Prior to this, he spent 18 years with Bell & Howell Company in Avrit of senior financial positions. He served as Senior Vice President of Finance - Worldwide for the Company's Publication Systems and Mail and Messaging Technology businesses.

Tom holds a B.S in Business Administration from Susquehanna University and an MBA in Finance from DePaul University.

GREGORY C. FISHER

Senior Vice President/Controller

Gregg joined D&B Plan Services in February 1990. He served as Assistant Controller and Controller as the company was sold by D&B to HealthPlan Services in 1994,went public in 1995, acquired Consolidated Group, Inc. and Harrington Services Corporation in 1996 and acquired CENTRAHealthPlan LLC and National Preferred Provider Network,Inc. in 1998. Gregg was appointed Senior Vice President in February 2000

Gregg worked from 1982 to 1984 a Staff Accountant for the regional accounting firm of Baird, Kurt and Dodson and from 1984 to 1990 as Controller of Security Tag Systems, public company that manufactured electronic detection equipment.

A graduate of the University of Arkansas in 1982, Gregg holds his BS BA in Accounting. He is currently a member of the American Institute of Certified Public Accountants and the Florida Institute of Certified Public Accountants.

SUE HARDT

Senior Vice President, Human Resources

Sue brings over 21 years of strategic and operational human resource leadership to Healthplan Holdings, with experience in a broad range of businesses and operating environments. Before joining in May 2006, she was a Vice President for Danka Office Imaging, where she was credited with elevating human resources productivity and quality for the company. Prior to that, Sue was a Senior Vice President and global leader in human resources for Kemper Insurance Companies of Long Grove, Illinois. Additionally, she has held distinctive titles for industry giants Alberto Culver Company and Elkay Manufacturing.

Sue credits her success in the corporate world to the operation of her own successful human resource company for over 11 years, which gives her the ability to see issues from multiple perspectives and develop effective, real world solutions.

Her formal education includes a Master of Science degree from Northern Illinois University. Sue is also active in many professional organizations.

HPS administers over One Million members through small business owners, sole proprietors and individuals. HPS' core customer base includes many of the largest companies insuring the small business and individual market place.
